Inspector Notes

An unannounced monitoring inspection was initiated on 04/29/2024 at 12:40 pm and concluded on 04/29/2024 at 2:50 pm.

At the time of the tour there were 169 children in care with 21 staff supervising. The children in care ranged in age from infant - four years. Classrooms were reviewed during nap time. Diapering and feeding was observed in the infant classrooms. Medication and emergency supplies additionally reviewed.

Records were reviewed for nine staff and four children.

Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued to the program. These violations were reviewed with the program director at the conclusion of the inspection.

Standard 8VAC20-780-160-B
Based on record review and interview, the center failed to ensure that documentation shall include the results of the tuberculin screening for staff.

Evidence:
1. The TB screening completed on 08/21/2023 for staff 1 lacked the results of the screening.
2. The program director verified this was the only documentation on file for staff 1.
Plan of Correction: The director stated that staff 1 would be asked to reach out to her health care provider for documentation of the results of the TB screening.
Standard 8VAC20-780-270-A
Based on observation, the center failed to ensure that outside areas of the center shall be maintained in a safe condition.

Evidence:
1. Approximately 38 bolts were observed protruding from underneath the rock climber outdoor play structure.
a. Children are able to walk/play under this structure and these exposed bolts are a potential protrusion hazard.
Plan of Correction: The director stated that the bolts had been covered however, the covers had come off. The bolts will be reviewed and covers replaced.
Standard 8VAC20-780-440-E
Based on observation, the center failed to ensure that there shall be at least 12 inches of space between occupied cots and rest mats.

Evidence:
1. 6 occupied cots in room 4 and 8 occupied cots in room 11 lacked 12 inches of space as each mat was placed directly beside another occupied rest mat.
a. Nap time was reviewed at approximately 1:00pm.
Plan of Correction: The director stated that all staff will be reminded to place rest mats at least 12 inches apart.
