Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 5/27/2021 and concluded on 5/28/2021. The Provider was contacted by telephone to initiate the inspection. There were 2 children present with 1 caregiver. The inspector emailed the Provider a list of items required to complete the inspection. The inspector reviewed 2 children?s records and 2 caregiver records submitted by the Provider to ensure documentation was complete.

The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the facility.

The licensing inspector has reviewed with the Provider COVID-19 Essential Guidance for Child Care programs.

Standard 22VAC40-111-180-A
Based on record review and interview, the provider did not obtain a subsequent tuberculosis (TB) screening for each caregiver every two years.
Evidence:
1. Caregiver #1 most recent TB screening is dated 4/25/2018.
2. Caregiver #2 most recent TB screening is dated 4/8/2019.
3. The provider confirmed that the TB screenings were expired and they had not been updated.
Plan of Correction: The TB tests have been scheduled and will be done 6/9/2021.

Based on record review, children's records were not kept up-to-date.
Evidence:
1. Child #1's most recent documented review of contact information is dated 2/11/2020.
2. Child #2's most recent documented review of contact information is dated 1/4/2020.
