Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 5/27/2021 and concluded on 5/28/2021. The Provider was contacted by telephone to initiate the inspection. There were 2 children present with 1 caregiver. The inspector emailed the Provider a list of items required to complete the inspection. The inspector reviewed 2 children?s records and 2 caregiver records submitted by the Provider to ensure documentation was complete.
The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the facility.
The licensing inspector has reviewed with the Provider COVID-19 Essential Guidance for Child Care programs.

2. Caregiver #2 most recent TB screening is dated 4/8/2019.
3. The provider confirmed that the TB screenings were expired and they had not been updated.

2. Child #2's most recent documented review of contact information is dated 1/4/2020.
Evidence: Child #1's record did not contain a street address for their second emergency contact.
