Inspector Notes

An unannounced monitoring inspection was conducted on 12/06/2022 between the hours of approximately 12:50 p.m. and 1:40 p.m. There was 1 child present with the Provider for a total of 2 points within the supervision guidelines. The child was observed reading a book and engaging in free play. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and caregiver records, fire drill log, menu, and attendance were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22.1-289.058
Based on observation and caregiver interview, the provider did not equip the home, that was built before 2015 and serves preschool-age children, with at least one carbon monoxide detector.

Evidence: When asked about the location of the carbon monoxide detector, Caregiver #1 stated that there is not a carbon monoxide detector in the home.
Plan of Correction: I will go to Walmart and get one.
Standard 8VAC20-800-320-B
Based on observation, child-resistant protective covers were not installed on all unused electrical outlets accessible to children under five years of age.

Evidence:
1. There were two uncovered electrical outlets next to the patio doors, accessible to Child #1 (age: 2 years).
2. There were two uncovered electrical outlets next to the toddler bed, accessible to Child #1.
Plan of Correction: The big kids take them out to plug in their chargers. I will cover them again.
Standard 8VAC20-800-580-C
Based on caregiver interview, daily age-appropriate activities did not include opportunities for vigorous outdoor play daily.

Evidence: Caregiver #1 stated that the children have not been outside to play since the summer.
Plan of Correction: I will start taking them to the playground.
Standard 8VAC20-800-690-D
Based on observation and caregiver interview, the diapering surface was not nonabsorbent and washable or cleaned and sanitized after each use.

Evidence:
1. Caregiver #1 stated that they change Child #1?s diaper on the bathroom rug located inside the bathroom used by the children.
2. The fabric bathroom rug was absorbent.
3. The diapering surface was not cleaned and sanitized after each use.
Plan of Correction: I will use the diaper changing pad.
