During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Inspector Notes

An unannounced, on-site monitoring inspection was conducted on March 31, 2026. The on-site inspection began at 9:15 a.m. and ended at 10:00 a.m. The inspector reviewed compliance in the areas listed above. There was 1 child present and 2 staff. The inspector reviewed 1 child?s record on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-550-D
The center shall implement a monthly practice evacuation drill. There was no documentation of an evacuation drill in July 2025, August 2025, September 2025, October 2025, November 2025, December 2025, January 2026, February 2026, or March 2026.
Plan of Correction: Emergency drills will resume this month.
Standard 8VAC20-780-580-L
Parental permission for transportation and field trips shall be secured before the scheduled activity. Center staff said they were going to take a walking trip to a local playground. There was no written permission from the parent on file.
Plan of Correction: A field trip permission form will be provided to the parent.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which contains required information. Child 1 did not have the address for 1 parent, the addresses for the emergency contacts, or documentation of any know allergies.
Plan of Correction: The missing items will be requested from the parent.
