Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was completed on May 28, 2021. The provider was contacted by telephone (video call) to initiate the inspection. There were 11 children present and in the care of the provider and two assistants. The inspector was able to view all required documentation. A virtual tour of the children in care was conducted. The provider's record and the records of two caregivers and three children's records were reviewed. Information gathered during the inspection determined non-compliances with applicable standards or law and the violations were documented on the violation notice issued to the facility.
The Licensing Inspector reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Violations
3Evidence:
1. The record for Child #1 did not contain written authorization from the parent stating that should an emergency occur and the parent cannot be located, the provider may seek medical care for the child. The record did not contain signed acknowledgment that the parent had received notice as to whether or not there was liability insurance in force on the family day home. Date of enrollment for Child #1 was 11/23/2020.
2. The record for Child #3 did not contain written acknowledgement that the parent was provided information regarding the family day home's emergency preparedness and response plan. Date of enrollment for Child #3 was 04/28/2021.
3. The provider confirmed these items were missing from the files of Child #1 and Child #3.
Evidence:
1. The first aid kit did not contain adhesive first aid tape.
2. The provider confirmed there was no first aid tape in the kit.
Evidence:
1. The record for Caregiver #3 did not contain the results of a search of the central registry. Date of hire was 12/20/2020.
2. The provider confirmed she had not received results from the search and she failed to follow-up with the Office of Background Investigations.
