Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on November 4, 2021. The provider was contacted by telephone and a virtual inspection was conducted. In the care of the provider and two assistants were 12 children, ranging in age from 5 months to 3 years (35 Points). The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, training, care of children, physical environment, nutrition and emergencies. Two child records, the provider's record and the records of one household member and two assistants were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations

1. The record for Child #2 did not contain a physical examination report. Date of enrollment for Child #2 was 09/15/2021.
2. The provider confirmed that she had not received a physical examination report for Child #2
Evidence:
1. The record for the provider and Household Member #1 did not contain a TB screening/test that had been completed within the last two years. The date of the most recent screenings for the provider and Household Member #1 was 07/02/2019.
2. The provider thought they had obtained updated screenings and lost the report, but when she called to obtain copies of the reports, she was told that the last TB that they had received was 07/02/2019.
Evidence:
1. The first-aid kit did not contain a First-Aid Manual.
2. The provider confirmed that she did not have a manual.
Evidence:
1. The record for Child #2 did not contain an immunization record. Date of enrollment for Child #2 was 09/15/2021.
2. The provider confirmed that the parent had not given her the child's immunization record.
