Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 12:25pm and 1:30pm. There were 2 classrooms observed with a total of 13 children with 2 staff within the supervision guidelines. The children were observed getting ready for nap. A complete inspection of the physical plant, children and staff records, fire drill log and emergency procedures were observed during this inspection. Violations were found during todays inspection and can be viewed on the Violation Notice.

Standard 8VAC20-780-240-C
Based on interview, orientation training does not include all of the required topics.
Evidence:
1. Orientation training does not include policies regarding lockdown procedures.
2. Orientation training does not include policies for preventing abusive head trauma and procedures for handling distraught children.
Plan of Correction: We will create procedures and get everyone trained.
Standard 8VAC20-780-245-L
Based on interview, there is not always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.
Evidence: The program director confirmed that none of the staff have current Daily Health Observation training.
Plan of Correction: We will get at least two staff trained.
Standard 8VAC20-780-530-A-1
Based on interview, there is not at least one staff in each classroom with current CPR certification.
Evidence: Staff #2 is the only staff member in the 'Juniors' classroom. Staff #2 does not have current CPR certification.
Plan of Correction: We will get them trained.

Based on interview, there is not at least one staff in each classroom with current first aid certification.
Evidence: Staff #2 is the only staff member in the 'Juniors' classroom. Staff #2 does not have current first aid certification.

Based on interview, the centers written emergency preparedness plan does not include facility readiness for lockdowns.
Evidence: When asked, the program director confirmed that the center has not yet established lockdown procedures in case of an intruder.
Plan of Correction: We will create procedures and train the staff.
