Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 08/07/2024, as a part of the licensure period. The on-site inspection began at 10am and ended at (12pm. The inspector reviewed compliance in the areas listed above. CDC: There were 8 children present and 2 caregivers; the point total was 23. The inspector reviewed 6 children?s records, 1 household record and 2 staff records on-site. This inspection included tour of the family day home and backyard, observations of staff and children, review of documentation and staff interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on child record review, documentation of a physical examination was not obtained within 30 days after the first day of attendance.
Evidence:
Child #6's first day of attendance was dated 03/04/2024. Documentation of a physical exam was dated 05/30/2024.
Plan of Correction: The parent emailed me the documentation but this information was not in the child's file.
Standard 8VAC20-800-90-A
Based on review of child records, documentation that a child has been adequately immunized was not obtained before the child began attending.
Evidence:
Child #4's first day of attendance was 03/04/2024. Documentation of immunizations was dated 05/30/2024.
Plan of Correction: The parent emailed me the documentation but this information was not in the child's file.

I will make sure all documentation is obtained before their first day.

Based on child record review, documentation of additional immunizations for a child under age 2 was not obtained.
Evidence:
Child #6 is under the age of 2 years. The most recent documentation of additional immunizations were dated 09/08/2023.
Plan of Correction: I will obtain the documentation of the child's 18 month appointment.
