Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A renewal inspection was initiated and and concluded on 8/18/21. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children (6 points) present, ranging in ages from 4 months to 4 years old, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Documents for new infant cribs added to the family day home were reviewed for compliance. There were no medications on site. A total of 1 child record, 2 caregiver's records and 2 household member records were reviewed. The children were observed playing with toys.

Evidence:
1. Caregiver # 1's first day of re-hire was 5/24/21
2. Caregiver # 1's record had fingerprints from May 2018 when she first started working with this family day home.
3. Caregiver # 1 stated that her initial employment at the family day home ended at approximately the beginning of 2019.
3. A time period of over 12 months had passed since Caregiver # 1's last day worked at the family day home, and new fingerprint results were not obtained prior to the new hire date of 5/24/21.
