Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A renewal inspection was initiated and and concluded on 8/18/21. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children (6 points) present, ranging in ages from 4 months to 4 years old, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Documents for new infant cribs added to the family day home were reviewed for compliance. There were no medications on site. A total of 1 child record, 2 caregiver's records and 2 household member records were reviewed. The children were observed playing with toys.

Standard 22VAC40-111-170-A
Based on records review, the provider did not obtain from each caregiver at the time of hire prior to coming into contact with children, a current Report of Tuberculosis (TB) Screening form.
Evidence:
1. Caregiver # 1's date of hire was 5/24/21.
2. Caregiver # 1's TB screening was dated 8/7/21.
Plan of Correction: I will remember to obtain TB results for new hires prior to start date.
Standard 22VAC40-191-60-C-2
Based on records review, the family day home did not obtain results of the Virginia Child Protective Search (CPS) for an employee within 30 days of employment.
Evidence:
1. Caregiver # 1's first date of hire was 5/24/21.
2. Caregiver # 1's CPS results were dated 7/13/21.
Plan of Correction: Forms will be submitted on time and follow ups will be completed and documented, if results do not arrive in time.
Standard 63.2-1720.1-B-2
Based on records review and staff interview, the family day home provider did not obtain results of a fingerprint based criminal history search for an employee before the first day of employment.
Evidence:
1. Caregiver # 1's first day of re-hire was 5/24/21
2. Caregiver # 1's record had fingerprints from May 2018 when she first started working with this family day home.
3. Caregiver # 1 stated that her initial employment at the family day home ended at approximately the beginning of 2019.
3. A time period of over 12 months had passed since Caregiver # 1's last day worked at the family day home, and new fingerprint results were not obtained prior to the new hire date of 5/24/21.
Plan of Correction: New fingerprint results will be obtained and a follow up will be conducted with the inspector.
