Inspector Notes

An unannounced renewal inspection was conducted today from 10:40 am to 12:20pm. There were 4 children (12 points) with two staff. The children were observed playing with toys, watching television and eating lunch. The lunch menu included chicken fingers, rice, peaches, salad and milk. There were no children with allergies or medications. Attendance records, emergency drill records and supplies were current. Areas of non-compliance are listed in the violation notice. Standard 22.1-289.035-B-4
Based on staff interview and record review, the provider did not obtain results of a fingerprint based criminal record checks and an out of state sex offender search for an employee prior to the first day of employment.
Evidence:
1. Staff # 1's stated that her first day of employment was 7/25/22. Staff # 1 also stated that she moved from New Jersey to Virginia in 2020.
2. An out of state sex offender search was not completed for Staff # 1 prior to her first day of employment.
3. Staff # 1's fingerprint results were obtained on 7/27/22.
Plan of Correction: I will make sure that fingerprinting will be completed prior to the first day of employment.
Standard 8VAC20-770-60-B
Based on records review and staff interview, the provider did not obtain a sworn disclosure statement (SDS) for a new employee prior to the first day of employment.
Evidence:
1. Staff # 1 stated that her first day of employment was 7/25/22.
2. Staff # 1's SDS was dated 7/28/22.
Plan of Correction: The sworn statement will be completed for any new hires prior to the first day of employment.
Standard 8VAC20-800-100-A
Based on records review, the provider did not obtain documentation of a physical examination within 30 days after the first day of attendance.
Evidence: Child # 2's record did not have documentation of a complete physical examination.
Plan of Correction: The missing information will be obtained from the parents.
Standard 8VAC20-800-680-B
Based on observation, caregivers did not ensure that children's hands are washed with liquid soap and warm running water prior to eating.
Evidence: Children did not wash hands with warm water and soap prior to eating lunch. They went directly from their activities to eating lunch.
Plan of Correction: I will make sure that we wash children's hands before all meals and snacks.

Based on records review, the provider did not obtain additional immunization records every six months, for children under the age of 2 years.
Evidence: Child # 1 's (13 months) record had the most recent immunization record from September 2021. Additional immunization updates were not obtained after that date.
Plan of Correction: The missing information will be obtained from the parents.
