Inspector Notes
An unannounced inspection was conducted on 12/6/22 from 10:15 am to 12:00pm, in response to a complaint received by the department on 11/17/22 related to the health of the provider, and the care and supervision of children. Two caregivers were interviewed, 2 children's records, attendance records and 3 caregiver's records were reviewed. Based on the preponderance of evidence the complaint was valid. Violations related to and not-related to the complaint have been listed in the Violation Notice. 2Standard 8VAC20-800-150-C
Based on staff interview, as assistant 18 years of age or older was left alone with children for more than two hours per day.
Evidence:
1. The provider was unavailable to work from 9/12/22 through 9/30/22.
2. Caregiver # 1 and Caregiver # 2 were the primary caretakers for the children that were attending the family day home during these days.
3. Caregiver #2 stated that from approximately 9:30 am to 1:00pm (3.5 hours), Caregiver # 2 would have to leave, and Caregiver # 1 was alone with the children in care.
Evidence:
1. The provider was unavailable to work from 9/12/22 through 9/30/22.
2. Caregiver # 1 and Caregiver # 2 were the primary caretakers for the children that were attending the family day home during these days.
3. Caregiver #2 stated that from approximately 9:30 am to 1:00pm (3.5 hours), Caregiver # 2 would have to leave, and Caregiver # 1 was alone with the children in care.
Plan of Correction: It was due to unforeseen circumstances that the provider was not available and Caregiver 1 was helping at the daycare. I will make sure that proper arrangements are made to maintain compliance.

Based on records review, the provider did not obtain at the time of hire and prior to coming into contact with children, a Tuberculosis Screening form for each caregiver.
Evidence:
1. Caregiver # 1's first day of employment was 10/1/22.
2. Caregiver # 1's TB screening was dated 10/14/22.

Plan of Correction: I will make sure that for any new employees, I obtain the TB screening prior to hem coming in contact with the children.
