During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 2/26/2025. The on-site inspection began at 12:35pm and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There was 1 child present who was the Provider's grandchild (1 point) and 1 caregiver with a total of 0 points. The inspector reviewed 2 children?s records and 3 caregiver/household member records on-site on 2/26/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with
applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3Child #1 and Child #2 did not have documentation of a physical examination on file at the time of the inspection. They have been enrolled for 3 months.
Child #1 and Child #2 were both missing documentation of proof of identity verification in their child files. They have both been enrolled for 3 months.
Child #1 and Child #2 did not have documentation of immunizations on file at the time of the inspection. They have been enrolled for 3 months.
