Inspector Notes
An unannounced inspection was completed on 07/21/2025 in response to a complaint received by the licensing office relating to allegations of Staffing and Supervision.
The inspector reviewed 2 staff records. This inspection included: document review and interviews.The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to (area or part of standard) and are documented on the violation notice issued to the program.

All staff are required to submit repeat background checks every five years following the last checks. Staff #2's last fingerprint was dated 01/21/2020, last central registry check was dated 02/13/2020 and last sworn statement of affirmation was dated 01/15/2020.
Plan of Correction: This was an oversight. The staff will complete a sworn statement of affirmation by the end of the day, the central registry check will sent out and fingerprinting will be scheduled as soon as possible.
