Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated on 08/09/2021 and concluded on 08/12/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present, ranging in ages from 3 years to 5 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the provider did not obtain for each caregiver current Report of Tuberculosis (Tb) Screening form, every two years from the date of the first screening.
Evidence:
1) The most recent Tb screening report in the record for Staff #1 was dated 08/08/2019. The most recent Tb screening report in the record for Staff #2 was dated 08/09/2019.
Plan of Correction: Staff #1 and Staff #2 shall obtain current Tb screening reports.

Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #2 did not contain address and telephone number a second custodial parent's place of employment.
Plan of Correction: I will contact the parent to provide the information.
