Inspector Notes

A renewal inspection was conducted on 08/21/2023 between the hours of approximately 9:00 am and 9:50 am. There were 7 children present, ranging in ages from 11 months to 4 years, with 2 staff supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-800-280
Based on observation, potentially poisonous substances and supplies, were not stored away in areas inaccessible to children.
Evidence:
1) On the date of inspection, the following items were observed on shelves that were not high enough for them to be inaccessible to children: one bottle of hand sanitizer, a container of Clorox wipes and a can of Lysol spray.
Plan of Correction: These supplies will be kept at a height that isn't accessible to children.
Standard 8VAC20-800-60-B
Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain the policy number of the child's medical insurance, and the first date of attendance.
Plan of Correction: Child's mother was contacted for policy information.
Standard 8VAC20-800-780-B
Based on record review, the provider did not review with the parent, annually, the emergency contact information to ensure the information is correct and obtain the parent's signed acknowledgment of the review.
Evidence:
1) The most recent review of the emergency information in the record for Child #1 was dated 06/21/2020 and for Child #2 was dated 02/01/2022.
Plan of Correction: Both children's emergency contact information was reviewed and acknowledged by parents on 08/23/23.

Based on observation, the provider did not ensure to maintain the temperature at or below 40?F in refrigerator compartments.
Evidence:
1) On the date of inspection, the thermometer in the fridge compartment read approximately 50?F
Plan of Correction: Refrigerator's temperature was adjusted.
