During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on February 5, 2025, as a part of the licensure period. The on-site inspection began at 8:40 a.m. and ended at 10:00 a.m. The inspector reviewed compliance in the areas listed above. There were 9 children present and 2 caregivers; the point total was 27. The inspector reviewed 5 children?s records, and 4 caregiver/household members records on-site. This inspection included document review, tour of the home, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 12, 2025. A POC submitted after this date will not appear on the public website.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Caregiver #3, who had been employed for over five months, did not have a completed national criminal background check.
Plan of Correction: Background check form was mailed in September 2024. I'll get in contact with the state.

The provider must request a search of the central registry prior to the caregiver's first day of employment.

The central registry request for Caregiver #3, who had been employed for over five months, was requested 33 days after employment.
Plan of Correction: Central registry results will be placed in the file for Caregiver #3.

REPEAT VIOLATION
Providers must maintain a record for each caregiver that contains the required information.

The record for Caregiver #2 did not contain verification of age and documentation of the required education and training. The record for Caregiver #3 did not contain verification of age, the name, address, and telephone number of a person to be notified in an emergency, documentation that two or more references were checked before employment, documentation of the required education and training, and documentation of a tuberculosis screening.
Plan of Correction: I will gather these missing documents for Caregivers 2 and 3.
Standard 8VAC20-800-170-A
Providers must obtain a current tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children.

There was no tuberculosis screening for Caregiver #3, who has been caring for children for over five months.
Plan of Correction: Caregiver #3 will have this done this month.

The provider must orient assistants by the end of their first week of assuming job responsibilities.

Caregiver #3 provided care for children for over five months without receiving orientation.
Plan of Correction: Orientation for Caregiver #3 will be done this month.

Poisonous materials shall be stored in an area inaccessible to children.

Poisonous cleaning chemicals were in an unlocked lower cabinet under the sink in the main room with children present.
Plan of Correction: I'll keep the cabinet locked with child-proof straps/locks.

Infant carrier seats shall be used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened.

Child #1, 4 months old, was observed unbuckled and asleep in a car seat, which was placed inside of a crib.
Plan of Correction: Child #4 was in the process of being moved to the crib. I'll make sure to follow manufacturer's guidelines.
