Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 12/17/2021 and concluded on 12/21/2021. The director was contacted by telephone to initiate the inspection. There were 8 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the licensee did not ensure staff completed tuberculosis screening prior to coming in contact with children or submitted at the time of employment.

Evidence:
The director was unable to provide documentation of staff #1 negative tuberculosis screening. Staff #1 hire date was November 29, 2021 . Staff #1 was working during the inspection on December 17, 2021.
