Inspector Notes

An unannounced, on-site monitoring inspection was initiated on September 9, 2024 and
completed on September 10, 2024 as a part of the licensure period. The on-site inspection began at 10:11 am and ended at 12:48 pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 7 staff. The inspector reviewed 5 children's records
and 3 staff records electronically on September 10, 2024. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or
law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Based on review of staff record and interview, the center did not ensure to obtain documentation of a tuberculosis screening every two years.

Evidence:
In Staff#1's record (date of hire: 05/21/21) the last dated tuberculosis screening was 05/19/21.

In Staff#2's record (date of hire: 03/07/22) the last dated tuberculosis screening was 03/04/22.

Based on staff record review and interview, the center did not ensure to complete orientation training prior to staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

Evidence:
In the boys/girls bathroom, two out of the four sinks were not operable, one with an out of order sign on it.
The playground area grass was observed uncut and accessible to children during outdoor activity.

Based on observation, the center did not ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the twos classroom, there was a bottle of hand sanitizer on the desk with the labels "keep out of reach" and "warning" accessible to the children in care.
