Inspector Notes
An unannounced, on-site monitoring inspection was initiated on September 9, 2024 and
completed on September 10, 2024 as a part of the licensure period. The on-site inspection began at 10:11 am and ended at 12:48 pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 7 staff. The inspector reviewed 5 children's records
and 3 staff records electronically on September 10, 2024. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or
law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 19, 2024. A POC submitted after this date will not appear on the public website

In Staff#1's record (date of hire: 05/21/21) the last dated tuberculosis screening was 05/19/21.
In Staff#2's record (date of hire: 03/07/22) the last dated tuberculosis screening was 03/04/22.
Interview with staff confirmed the available documentation for review during the inspection.
Evidence: In Staff#3's record, there was no documentation found of completion of orientation. Interview with staff confirmed there was no documentation available for review during the inspection.
Evidence: The center's annual fire inspection was dated 04/13/2023 during the inspection.
Evidence:
In the boys/girls bathroom, two out of the four sinks were not operable, one with an out of order sign on it.
The playground area grass was observed uncut and accessible to children during outdoor activity.
Evidence: In the twos classroom, there was a bottle of hand sanitizer on the desk with the labels "keep out of reach" and "warning" accessible to the children in care.
