Inspector Notes

An unannounced monitoring inspection was conducted on 1/19/23 from 11:30am to 1:35pm with the Director and Assistant Director. There were 62 children in care, ranging in age from six-months to five-years-old, supervised by 11 staff. The children were observed eating lunch and preparing for nap. Six child records and five staff records were reviewed. The Center has 21 staff with current certification in CPR and First Aid, and 21 staff trained in Daily Health Observation Training. One medication and authorization form were reviewed and the center has seven staff current in Medication Administration Training (MAT). The attendance, menu, required postings, and emergency drill log were reviewed. They playground was observed. The most recent Fire Inspection on file was dated 1/17/23 and the most recent Health Inspection on file was dated 3/4/22. Lunch served was chicken and rice with peas, mandarin oranges, and milk. Based on observation and review of documentation, the center did not ensure that for each group of children, a written record of daily attendance was maintained that documented the arrival and departure of each child in care as it occurred. Evidence: On 1/19/23 at approximately 11:32am, four infants were observed in the Infant classroom. The attendance contained documentation of five infants present. Staff reported that an infant was moved to the Older Infant classroom at approximately 9:00am, but the attendance was not updated.
Plan of Correction: All staff were trained on attendance procedures on 1/19/2023 at a staff meeting.
