Inspector Notes

An unannounced monitoring inspection was conducted with the local agency on 05/04/2023 from 1:00pm to 1:50pm as a result of a self-reported incident involving an injury to a child. There were 7 children present with the Provider and all of the children were laying down for nap time at the time of inspection. The program area, attendance, 1 child record, and documentation were reviewed. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-800-50-A
Description: Based on review of the written attendance record and interview with the Provider, the Provider did not ensure they maintained a written record of children in attendance each day.

Evidence:
During the inspection in was determined there were 7 children present, but the written record reflected there were 5 children present.
Plan of Correction: I'll make sure to have everyone's attendance next time.
Standard 8VAC20-800-60-A
Based on review of 2 child records, the provider did not ensure that that the record for each enrolled child included the name, address and telephone number of two designated persons to contact in case of emergency if the parent cannot be reached.

Evidence: The record for Child #1 did not contain a 2nd designated person, address, and telephone number to contact in case of emergency.
Plan of Correction: The parent did not have any other person to put in there next time I will not have that kind of parent.
