Inspector Notes
An unannounced monitoring inspection was conducted with the local agency on 05/04/2023 from 1:00pm to 1:50pm as a result of a self-reported incident involving an injury to a child. There were 7 children present with the Provider and all of the children were laying down for nap time at the time of inspection. The program area, attendance, 1 child record, and documentation were reviewed. Areas of non-compliance are identified in the Violation Notice.

During the inspection in was determined there were 7 children present, but the written record reflected there were 5 children present.
Evidence: The record for Child #1 did not contain a 2nd designated person, address, and telephone number to contact in case of emergency.
