Inspector Notes

An unannounced monitoring inspection was conducted today between 9:11am and 10:20am. There were 9 children present with 2 caregivers supervising within the supervision guidelines. The children were observed playing leggos, trucks, a push toy, sliding, sleeping and being fed. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas administration, physical plant, staffing and supervision, programing, fire drill log, attendance and emergency procedures. A total of 4 child records and 2 caregiver?s records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the programs. Based on record review, it was determined documentation of additional immunizations was not obtained every six months for children under the age of two years.
Evidence: Child #1 (DOB 12/16/2020) most recent documented immunization record is dated 06/21/22.
Plan of Correction: I will tell my parents tonight that I need an updated immunization record.

Based on observation and interview, the menu for the current one-week was not given to parents or posted or placed in an area accessible to parents.
