An unannounced, on-site monitoring inspection was initiated and completed on 03/03/2026, as part of the licensure period.

The on-site inspection began at 11:30 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 3 caregivers; the point total was 27. The inspector reviewed 2 children?s records and 5 caregiver/household members records on-site.

This inspection included document review and tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 03/10/2026. A POC submitted after this date will not appear on the public website.

The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years.

Child-resistant protective covers shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

One unused electrical outlet did not have a protective cover installed. Children were eating lunch in the area were the unused electrical outlet was located.
