Inspector Notes
An unannounced monitoring inspection was conducted on October 17, 2022. Upon arrival at 12:10 PM there were a total of 4 children under the care of 1 caregiver present. The Licensing Inspector reviewed 4 children?s records and 1 household/staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

4EVIDENCE:
A carbon monoxide detector was not installed in the home.
EVIDENCE:
Using a food/liquid thermometer, Licensing Inspector observed the hot water temperature, in the bathroom sink used by the children in car, registered 122.9 Degree F. Hot water temperature was not maintained within a range of 105 degree F to 120 degree F.
EVIDENCE:
Licensing Inspector observed the Provider placed a child in an infant crib for purposes other than sleeping. The Child was observed playing with manipulative toys while in the crib. Infant cribs was used as a play space.
EVIDENCE:
Licensing Inspector reviewed 4 children records. Record for Child #4, who is under the age of 2 years old, did not include documentation of updated immunizations every 6 months.
