Inspector Notes
A mandated monitoring inspection was conducted on 10/4/2023 from 1:22 pm to 2:35 pm. Two children were in care at the time under the care of one caregiver. The Licensing Inspector reviewed two children?s records, one staff record, and other program records. The LI conducted a walk-through of the house.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

3EVIDENCE:
The diaper mat had a rip in it, making it no longer non-absorbent.
EVIDENCE:
The record for child 1 has a documented start date of 4/5/2021 and has no documentation to show that the parent was provided with information regarding the administration of medication.
EVIDENCE:
The record for child 1 has no documentation that the parent has given permission for the child to be taken off the premises. An interview with staff 1 reveals that children in care regularly go on neighborhood walks.
