During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on and completed on 2/27/2025. The on-site inspection began at 12:40pm and ended at 2:10pm. The inspector reviewed compliance in the areas listed above. There were nine children present and two caregivers; the point total was 22. The inspector reviewed five children?s records and five caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-250-E
Latex gloves, balloons, and empty plastic bags large enough for a child's head to
fit inside shall be inaccessible to children under five years of age. Latex balloons were found in an area accessible to children under five years of age.
Plan of Correction: Latex balloons were removed during the inspection and the provider understands they cannot be accessible for children under five years.
Standard 8VAC20-800-510-D
Rest mats that are used must have at least an inch of cushioning. The rest mats being used during naptime did not have at least an inch of cushioning.
Plan of Correction: The provider will ensure that rest mats have at least one inch of cushion.
Standard 8VAC20-800-530-A
Cribs, cots, rest mats, and beds when being used for sleeping or napping by children other than infants shall have linens consisting of a top cover and a bottom cover or a one-piece covering that is open on three edges. There only top linens were being used by children during naptime for the nap mats.
Plan of Correction: The provider will ensure that children have a top and bottom linen during naptime.
Standard 8VAC20-800-580-A
In order to promote the child's physical, intellectual, emotional, and social
well-being and growth, caregivers shall provide needed help, comfort, and support. An infant was crying in their crib in another room for over seven minutes and the caregivers did not provide needed comfort and support until the inspector asked them to give the child some care.
Plan of Correction: The child was given comfort and the provider understands that crying infants require comfort when settling to sleep.
Standard 8VAC20-800-60-B
Repeat Violation

Children's records are required to contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. The record for Child #1 and Child #3 only contained the information of one designated person to contact in case of an emergency if the parent cannot be reached. Both children were present during the inspection and had been in care for over a month.
Plan of Correction: The provider will obtain a second emergency contact for the children.
Standard 8VAC20-800-80-A
Within seven business days of the child's first day of attendance, the provider shall obtain from the parent verification of the identity and age of the child and name and location of previous day care programs. This information was not available for five children who had been provided care in February 2025.
Plan of Correction: The provider will obtain proof of identification for the children.
