During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on 4/28/2025 and completed on 5/12/2025 in response to a self-report received by the licensing office on 4/23/2025 relating to supervision of children. The inspector reviewed compliance in the areas listed above. The inspector reviewed six children?s records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 5/23/2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-800-30-E
The provider shall ensure that the home's activities, services, and facilities are conductive to the welfare of the children in care. The provider failed to ensure that the home's activities, and services were conductive to the welfare of children when Child #1 (3 years old) was able to leave the neighborhood playground unnoticed and walk at least 0.2 miles unsupervised, crossing at least two roads with 25MPH speed limits, and was about to attempt to cross a four lane road with a 45MPH speed limit when they were found by a member of the community.
Plan of Correction: Policy change: Program activities will no longer include field trips or outing outside of daycare center gates.
Parents have been notified to require children to hold hands with parents or walk carefully next to parents during drop off and pick up times due to car drop off being on a street.
Standard 8VAC20-800-560-B
Caregivers shall actively supervise each child during outdoor play. On 4/22/2025, while one caregiver was helping a child onto a swing, and the other was observing the children playing underneath a slide, the caregivers did not actively supervise each child during outdoor play when Child #1, age 3, was able to leave the neighborhood playground and be out of the care of the provider for 41 minutes. The child walked away from the playground and was in the process of attempting to cross a busy four lane street with a 45 MPH speed limit when they were found by a member of the community who brought the child to a nearby childcare center. The community member and the child were unknown to each other. The child was alone for an estimated seven minutes before they were found by the member of the community. Emergency services were contacted, and the police coordinated the reunification of the child with the provider.
Plan of Correction: A caregivers main responsibility is to provide consistent and high quality care for children under their supervision.
Due to this incident both caregivers have been terminated from the daycare center.
All future employees and children will be under camera supervision in play areas as well as gated premises when playing outside. Any incident must be reported to provider immediately. Camera footage of any incidents will be available to parents and inspectors.
Standard 8VAC20-800-650-E
The provider shall notify the parent immediately when the child is lost or missing. The provider did not contact the parents until their child had been missing for 40 minutes.
Plan of Correction: Parents will be called immediately when any issue arises with their child.
Standard 8VAC20-800-850-A
The provider shall report to the department within 24 hours when a child is lost or missing and the local authorities have been contacted for help. The provider did not report to the department until approximately 29 hours after the child was missing. Local authorities were contacted for help when Child #1 went missing.
Plan of Correction: Department policies will be reviewed and checked after any incident arises. All rules and regulation that are provided by the department of education will be printed out and readily available to provider and caregivers in care of any questions or concerns.
Standard 8VAC20-800-980-A
General written permission shall be obtained from the parent of each child for the provider to take a child off the premises of the family day home. Children #1-#6 walked to a playground in the neighborhood, with two assistant staff on 4/22/2025, but there was no written permission for any of the children to be taken off the premises.
Plan of Correction: New playground permission forms will be drafted and signed for playground and play structures as well as any outdoor activities, including water, located on daycare property upon completion of daycare's renovation. Parents submitted forms will be reviewed until all required forms are signed and discussed.
