During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on and completed on 9/3/2025. The Department issued a Special Order on May 5, 2025, to place the licensee on probation and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety as a result of high-risk violations. The on-site inspection began at 2:30pm and ended at 3:40pm. The inspector reviewed compliance in the areas listed above. There were six children present and two caregivers; the point total was 17. The inspector reviewed five children?s records and three caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3protective barriers at stair openings. There was a pressure mounted gate used as a protective barrier at the stair openings from the basement up to the first floor.
Rest mats that are used must have at least an inch of cushioning. The rest mats being used during naptime did not have at least an inch of cushioning.
