Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 1/29/2026. The Department issued a Special Order on May 5, 2025, to place the licensee on probation and require the licensee to contact parents, guardians, or other responsible persons regarding health
and safety as a result of high-risk violations. The on-site inspection began at 12:45pm and ended at 1:45pm. The inspector reviewed compliance in the areas listed above. There were seven children present and two caregivers; the point total was 16. This inspection included tour
of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
