Inspector Notes
An unannounced monitoring inspection was conducted from 1:15 - 2:45pm with the provider and her two assistants. There were 11 children in care, ranging from one month to three years of age, for a total of 36 points. Children were observed napping, waking, and playing with age-appropriate toys. Five child records, five caregiver/household member records, the emergency drill log, daily attendance, and the required posted information was reviewed. It was reported that there were no medications on site, however the provider has current certification in Medication Administration Training (MAT).

Based on observation, and an interview with the provider, not all children were provided an individual crib for resting/napping. Evidence: There was no crib assigned or available for a one-month old infant observed in care and a seven-month old (another child was using their crib.)

Based on observation, the provider failed to ensure that no soft bedding of any kind was used under or around infants including, but not limited to pillows, quilts and comforters. Evidence: there was a blanket hanging on the side of two different cribs that each had an infant resting/sleeping inside.

Standard 8VAC20-800-560-A
Based on observation, the provider failed to provide direct supervision of each child by monitoring each sleeping infant. Evidence: three infants were observed sleeping in rooms with doors closed. These infants were not in sight and hearing of the provider; the provider did not use a baby monitor; and the provider did not conduct an in-person observation of each sleeping infant at least once every 15 minutes as required.

Based on observation, the provider failed to ensure that children are not allowed to drink while lying down. Evidence: An infant was observed lying in a crib with a bottle of milk.
