Inspector Notes

An unannounced monitoring inspection was conducted on Monday, March 4, 2024, with the center director. There were 46 children present with 9 staff supervising. The inspection began at approximately 9:15 AM and concluded at 11:00 AM. Children were observed participating in circle time and interacting with staff. All areas of the center were observed including classrooms, bathrooms, and the playground. A total of 5 children records and 5 staff records were reviewed.

The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice to the provider.

Standard 8VAC20-780-130-A
(Repeat Violation) Based on review of children records, the center failed to obtain immunizations before the child began attending the center.

Evidence:
1. Child # 5?s record did not contain immunization records.
2. The director verified that immunizations were not received for child #5.
Plan of Correction: Plan to contact student with missing info.
Standard 8VAC20-780-140-A
Based on record review, the center failed to obtain documentation of a physical examination before the child?s first day of attendance or within 30 days after the first day of attendance.

Evidence:
1. Child # 5?s record did contain documentation of a physical examination. Child #5?s date of enrollment was 9/11/2023.
2. The director verified that a physical examination had not been received for child #5.
Plan of Correction: Plan to contact student with missing info.
Standard 8VAC20-780-260-B
Based on review of records and interview with staff, the center did not ensure that an annual health inspection report was provided.

Evidence:
1. The most recent health inspection was completed on 9/23/2022.
2. The director verified that a health inspection was not conducted in 2023.
Plan of Correction: Will call and schedule a health inspection today.

Based on record review and interview, the center failed to ensure that shelter -in- place procedures shall be practiced twice per year.

Evidence:
1. The emergency drill log for the center had documentation of one shelter-in-place practice drill for 2023. Last drill was documented on 2/24/2023.
2. The director confirmed that a second shelter-in-place drill had not been conducted.

Based on record review and interview, the center failed to ensure that a lockdown procedure shall be practiced annually.

Evidence:
1 The emergency log for the center did not have documentation of a lockdown drill conducted for 2023. The last lockdown drill was documented on 10/26/2022.
Plan of Correction: Scheduled a lockdown drill for March 19, 2024.

(Repeat Violation) Based on review of five (5) children records, the center failed to ensure that children?s records contained all required information.

Evidence:
1. The record for child #3 (date of enrollment 1/29/2024), did not contain an address for the second emergency contact listed.
2. The record for child #5 (date of enrollment 9/11/2023), did not contain and address for two (2) emergency contacts.
Plan of Correction: Will follow up with all missing contact addresses.
