Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).
A monitoring inspection was initiated on 10-6-21 and concluded on 10-13-21. The provider was contacted by telephone and a virtual inspection was conducted. There were nine children present, ranging in ages from 7 months to 3 years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

1. The TB test results on file for staff #1 & 2 were not repeated at the two year anniversary date. 5-9-19
Evidence:
The First Aid & CPR certification expired for both caregivers on 10-5-21.
