Inspector Notes

An unannounced monitoring inspection was conducted today. There were 7 children present (3 points) with 2 caregivers.
Children were observed resting quietly and playing with a variety of age appropriate toys and books.
Emergency evacuation drills, emergency supplies, first aid supplies, first aid/CPR certifications, fridge/freezer temperatures, posted documents and the menu are all in compliance.
3 caregiver/household member records and 4 child records were reviewed. See Violation notice for non-compliances cited.

Based on a review of records, one caregiver record a ne household member record did not contain documentation of a repeated nationwide fingerprint search within the allowable 5 year time frame.
Evidence:
1. Caregiver A's record did not contain a repeated nationwide fingerprint search result dated within the last 5 years. The results on file are 3/28/18. (expired)
2. Household member #1's records did not contain a repeated nationwide fingerprint search result dated within the last 5 years. The results on file are dated 4/1/18 (expired).
Plan of Correction: I will fill out the fingerprint form and get my fingerprinted.

Based on a review of records, one caregiver and one household member's records did not contain a current TB screening result every two years from the date of the last report.
Evidence:
