Inspector Notes

An unannounced monitoring inspection was conducted today from approximately 12:00pm-1:00pm. There were 12 children in the direct care and supervision of 3 caregivers. A sample of 3 children?s records, and 3 caregivers' records were reviewed. The children were observed napping. Zero new caregivers have been hired since the last inspection. Based on record review, the assistants' records did not contain required information.

Evidence:
Staff B and C did not have verification of age in their file.
Plan of Correction: I will get a copy of their identification card and add it to their file.

Based on interview and record review, staff did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
Staff A stated that she did not maintain proof of training documents for Staff A, B and C.
