Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

A monitoring inspection was initiated on 9/17/2021 and concluded on 9/22/2021. The Provider was contacted by telephone and a virtual inspection was conducted. There were 3 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, special care and emergencies and nutrition. A total of 2 child records 2 staff records and 1 household member record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the provider has not received a subsequent tuberculosis (TB) screening every two years.
Evidence: The providers most recent TB screening is dated 9/5/2019. The provider confirmed that they have not yet received an updated screening.
Plan of Correction: I have made an appointment to get a screening.
