A complaint was received in the licensing office on 11/18/2022 regarding supervision and assigned caregivers. On 11/21/2022 an on-site investigation was conducted. Interviews were conducted and records were reviewed. At the time of this investigation, the preponderance of evidence supports the allegation and the complaint is deemed valid.

Based on record review and interview, parents were not provided in writing the required information before each child's first day of attendance.
Evidence: Child #6s record contained a blank "Information for parents" form. The provider did not notify the parents the amount of time per week that an adult assistant or substitute provider instead of the provider is scheduled to care for the child and the name of
the adult assistant or substitute provider.

Plan of Correction: I always have the parents sign the forms first and then I fill the information in. I didn't know I needed to complete the form first. I will start giving families the form filled out before they sign.
