Inspector Notes
An unannounced renewal inspection was conducted today between 12:10pm and 1:30pm. There were 5 children present with the Provider and 2 assistants for a total of 12 points within the supervision guidelines. The children were observed resting during the inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. The provider stated that there were no medications kept on-site. Violations were found during today?s inspection and can be viewed on the Violation Notice.

5Evidence: When asked for todays attendance, the provider stated that she hadn't completed attendance that day.
Evidence: When asked for documentation of last weeks attendance, the provider stated that they do not keep their weekly attendance records.
Evidence: Child #1 (18 months old) was observed in a crib with a pillow.
Evidence: Child #3's record does not contain two emergency contacts, authorization for medical attention, documentation that the parents have been informed of the providers policies, emergency procedures or documentation of liability declaration.
Evidence: Child #3's record does not contain documentation of immunizations.
