Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and concluded on 5/24/21. The provider was contacted by telephone to initiate the inspection. There were 8 children (23 points) and 2 caregivers present. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, 2 caregiver's records and 2 household member's records submitted by the facility to ensure documentation was complete. Additional family day home records were also reviewed. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Standard 22VAC40-111-180-A
Based on record review, the provider did not obtain a subsequent Tuberculosis (TB) screening for each caregiver every two years from the last screening.
Evidence: Caregiver # 1's most recent TB screening was dated 10/20/18, and expired in 10/28/20.
Plan of Correction: The TB screening will be completed.
Standard 22VAC40-111-60-B
Based on records review, children's records did not contain complete information.
Evidence:
1. Child # 1's record was missing address information for the second emergency contact person; information for persons authorized to pick up; and the mother's work address and phone number.
2. Child # 2's record was missing address information for the second emergency contact person.
Plan of Correction: The information will be obtained and logged into the children's records.
