Based on a review of four children's records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child C (start date 2/22/22) and child D's (start date 6/6/22) record did not have documentation of a physical examination in them.

Evidence: Child A, B, C, and D's records were missing documentation that the parents received the policy regarding administration of medication from the provider.
Plan of Correction: The provider will print a copy of the sample form, provide the policy in writing to the parents, and place documentation that they received the policy in each child's record.
