Inspector Notes

An unannounced monitoring inspection was conducted on 4/26/2023 from 12:50 pm-2:30 pm. There were 7 children (19 points) directly supervised by 2 caregivers. The home, 2 caregiver records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping. Areas of non-compliance are identified in the Violation Notice. Based on record review and interview, three of four background checks were not repeated every five years.

Evidence:
1. The most recent central registry result on file for Household member #2 is dated 4/16/2018 and expired on 4/16/2023.

2. The most recent criminal history result on file for Caregiver #1 is dated 2/20/2018 and expired on 2/20/2023.

3. The most recent criminal history result on file for Caregiver #2 is dated 2/20/2018 and expired on 2/20/2023.
Plan of Correction: 1. We filled out a new application for the central registry.
2. We made an appointment for fingerprinting.
Standard 8VAC20-800-180-A
Based on record review and interview, the provider did not obtain updated tuberculosis (TB) tests or screenings for each household member every two years.

Evidence: Household member #2 tuberculosis test expired on 6/12/2022.
Plan of Correction: We made an appointment for (TB) test.
Standard 8VAC20-800-530-G
Based on observation and interview, the family day home failed to ensure that children under two years of age do not use pillows.

Evidence: Child #3 (DOB: 6/10/2021) was observed in their crib, laying on a pillow that is attached to a fabric mat, and blanket.
Plan of Correction: During the inspection I immediately removed the blanket from the crib. Child #3 on 6/10/2023 turns 2y.
