During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 12/12/2024 and completed on 12/12/2024. The on-site inspection began at 10:04 am and ended at 12:20 pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 13. The inspector reviewed 5 children?s records 2 caregivers, and 2 household member(s) records on-site. This inspection included document review, a tour of the home, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/20/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-590-B
An infant who falls asleep in a play space other than their own crib shall be moved promptly to his designated sleeping space if the safety or comfort of the infant child is in question.

During the inspection, Child #1 was found sleeping in an infant seat, with a blanket on top and a bottle in their mouth.
Plan of Correction: Corrected on the spot.

Child #1 was moved to their designated sleeping space upon correction.

Caregiver #2 was reminded of the orientation training they received.

Caregivers shall wash their hands with liquid soap and warm running water: After diapering

During the inspection, it was observed that Caregiver #2 did not wash their hands after diapering Child #4.
Plan of Correction: Corrected on the spot

Caregiver #2 washed her hands immediately after changing child #4 and was reminded of hand washing procedures regarding diaper change.

Caregivers shall ensure that children's hands are washed with liquid soap and warm running water: After diapering

During the inspection, it was observed that Child #4 did not wash their hands after having their diaper changed by Caregiver #2.
Plan of Correction: Corrected on the spot.

Caregiver #2 washed child #4's hands immediately after changing his diaper and was reminded of hand washing procedures regarding diaper change.
Standard 8VAC20-800-960-B
Infants who cannot hold their own bottles shall be picked up and held for bottle feeding. Bottles shall not be propped.

During the inspection, it was observed that Child #1's bottle was propped up with a blanket while sleeping in an infant seat.
Plan of Correction: Corrected on the spot.

The bottle was removed immediately.

Ensure that no child is left with their bottle in their mouth, regardless of whether the child is theirs or not. Additionally, caregiver #2 was reminded of the orientation training they received.
