Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 5/22/2025 and completed on 5/22/2025. The on-site inspection began at 9:45 am and ended at 10:40 am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 1 caregiver; the point total was 12. The inspector reviewed 4 children?s records, 1 caregiver record, and 3 household member(s) records on-site. This inspection included document review, a tour of the home, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The immunizations received shall be signed by a licensed physician, the physician's designee, or an official of a local health department.

The immunization record available for Child #1 was missing a physician's signature, the physician's designee, or an official of a local health department.
Plan of Correction: I informed the parents tha the doctor's office must stamp or sign the vaccination record.
Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for Household member #3 was not completed and was overdue by four days.
Plan of Correction: Made an appointment for household member at the clinic for a tuberculosis test on wenedsday, for when she returns this weekend
