Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment and shall have been completed within the last 30 days of the date of employment.
At the time of the inspection, Caregiver #2 had documentation of a negative tuberculosis screening dated approximately fifty days prior to the date of employment and had been employed for approximately four months.

Plan of Correction: I didn't think that having negative TB test results for caregiver #2 before starting work would a problem in the future. I well make sure to get them within the first 30 days of employment.

When meals or snacks are provided by the family day home, the menu for the current one-week period must: be dated, be given to parents or posted in an area accessible to parents; list any substituted food; and be kept on file for one week at the family day home.
At the time of the inspection on Wednesday, December 10, 2025, there was no menu available for the current one-week period beginning Monday, December 8, 2025. The provider later wrote and posted the menu during the inspection.
