Inspector Notes

A monitoring inspection was on conducted onsite on October 4, 2021 regarding a self reported child supervision incident. Staff interviews were conducted along with inspection of the physical plant.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-185-340-A
Based on a review of the center's policies and procedures and staff interviews, it was determined that when staff were supervising children, they did not always ensure their care, protection and guidance.
Evidence:
1. On September 30, 2021 at approximately 3:27pm to 3:30pm, Child #1, age 1 years old, was found unattended on the playground.
2. Child #1 was located by staff #4, approximately 20-24 feet from the closed classroom door they came out of.
3. Staff #1 was changing a diaper, staff #2 and #3 were cleaning up after snack. Staff #1, #2 and #3 in classroom were not aware child #1 was not in the class.
4. Staff #2 and #3 confirmed Child was last observed in the classroom at approximately 3:25pm to 3:28pm.
5. It was determined Child #1 pushed open the classroom door to the playground and proceeded to walk down the sidewalk in the enclosed playground area.
Plan of Correction: The exit door in the classroom room wasn't closing properly which permitted the child to open the door without assistance. This has already been repaired as facilities repairs were contacted the

Based on staff interviews, it was determined that the staff did not ensure that children under the age of 10 years old shall be within actual sight and sound supervision of staff.
Evidence:

Based on a review of the center's policies and procedures and staff interviews, it was determined that the licensee did not ensure that the center's own policies and procedures required by the standards, and the terms of the current license issued by the department were not followed.
Evidence:
1. The center's policies and procedures regarding supervision of children states that, "staff must know where each child is located and be aware of children?s activities at all times?.
2. On September 30, 2021 at approximately 3:27pm to 3:30pm, Child #1, age 1 years old, was found unattended on the playground.
3. Child #1 was located by staff #4, approximately 20-24 feet from the closed classroom door they came out of.
4. Staff #2 and #3 confirmed Child #1 was last observed in the classroom at approximately 3:25pm to 3:28pm.
