Based on interview with center director, the center did not have available a written record of children's serious and minor injuries in which entries are made the day of occurrence for review by the inspector.

Plan of Correction: Incident and accident reports were written and signed by parents but were misfiled and not readily available when requested. Center will review and implement an improved plan to ensure any future reports are readily available.
