Inspector Notes

An unannounced inspection was conducted today in response to a self-reported incident that was reported to the licensing office on 1/12/2024. . Interviews were conducted with staff relating to the incident. The information gathered during this investigation finds the self-report of a child being given the wrong bottle as valid. Violations related to this inspection can be viewed on the violation notice.

Standard 8VAC20-780-340-A
Based on interviews with staff, when staff was supervising children, they did not always ensure their care and protection.
Evidence
1. On January 12, 2024, staff #1 used approximately one ounce of child #1's bottle of breast milk to mix with child #2's cereal.

2. Staff #1 fed child #2 approximately 4 teaspoons of the cereal mixed with child #1's breast milk.

3. Staff #2 observed child #2 bottle still in the bottle warmer and stopped staff #1 from feeding child #2 any more of the cereal.

4. Staff #1 did not always ensure the care and protection of child #2.
Plan of Correction: We have retrained all staff on bottle feeding procedures.
We increased classroom observations in the infant classroom.
