During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on February 26, 2025, as a part of probation licensure period. As authorized by ? 22.1-289. 023 .B .1 of the Code of Virginia, place the licensee on probation for a period of six (6) months; and 2.As required by?? 22.1-289.01l.F, 22.1-289.023.A of the Code of Virginia, and8VAC20-820-120-E-6, General Procedures and Information for Licensure, the licensee shall post (i) the Notice of Intent dated May 15, 2024, (ii) any Violation Notice(s) attached to the Notice of Intent, and (iii) this Special Order in a prominent place at each public entrance of the licensed premises of Winwood Children's Center-856, 18792 Upper Meadow Drive, Lansdowne, Virginia 20176 for a period of twelve (12) months. The on-site inspection began at 11:35am and ended at 12:00pm. This inspection included document review and tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which is 3/12/2025. A POC submitted after this date will not appear on the public website.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

The Name to Face roster for Classroom #1 indicated there were 12 children in attendance but only 10 children were present. Staff #1 stated 2 children were moved to Classroom #2.

The Name to Face roster for Classroom #2 did not document the arrival of the 2 children.
For approximately 20 minutes, the record indicated there were 15 children in attendance when there were 17 present.
Plan of Correction: Staff were retrained on signing children in and out of Name to Face sheets.
