Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 08/26/2024 as a part of the licensure period. The on-site inspection began at 11am and ended at 1:10pm. The inspector reviewed compliance in the areas listed above. There were 33 children present and 8 staff. The inspector reviewed 4 children?s records and 4 staff records on-site. today. This inspection included documentation review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on child record review, documentation that one child received immunizations required by the State Board of Health was not obtained before the first day of attendance.
Evidence:
Documentation of immunizations for Child #4 who began care on 08/26/24 was not obtained.
Plan of Correction: Documentation of immunizations will be obtained and maintained in the child's file.
