During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/18/2026, as a part of the licensure period. The on-site inspection began at 10:15am and ended at 12pm. The inspector reviewed compliance in the areas listed above. There were 38 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

A cleaning agent was found on the counter, accessible to children in one classroom.
Plan of Correction: Staff will be reminded to lock all cleaning agents if not in use.

Substitute containers shall clearly indicate their contents and shall not resemble food or beverage containers.

An unlabeled spray bottle with fluid was found on the counter, accessible to children in one classroom.
Plan of Correction: The contents of this spray bottle will be determined and labeled.

Repeat
Over-the counter skin products shall not be kept or used beyond the expiration date of the product.

One over the counter skin product that expired six months ago, was found in one classroom.
Plan of Correction: The ointment will either be disposed of or returned to the parent today.

If diaper ointment or cream is used, written parent authorization noting any known adverse reactions shall be obtained.

Written authorization for one diaper ointment was not obtained for Child #1.
Plan of Correction: Discuss with parent whether a new ointment is needed and will obtain a written authorization.
