Inspector Notes
An unannounced monitoring inspection was conducted from 3:00 ? 3:45 pm with staff. There were 13 children, ranging from 5 years to 11 years of age, present with 2 staff supervising. Children were observed playing outside on the playground. Two children?s records and two staff records were reviewed off site at the Massad office on 5/11/22. Two medications were reviewed along with their authorization forms and one allergy care plan. Staff have current certification in Medication Administration Training (MAT), first aid and CPR. The attendance and written allergy list were reviewed and in compliance.
